Referrals and Eligibility

WVUH-East Home Health Care can provide your home health services regardless of where you were hospitalized. Our services are available to any Jefferson or Berkeley County resident who needs skilled care and finds it difficult to leave his or her home because of illness or injury. There are no age restrictions.

Referrals are accepted from any source. Once a referral is made, we will make arrangements for a free evaluation visit by a registered nurse to determine eligibility for admission and need for other services.

Home health care services will never be denied upon the basis of age, sex, race, color, creed, national origin or handicap.
