Memorial Gifts
Donations to either the City Hospital Foundation or Jefferson Health Care Foundation may be made in memory of a friend or family member.
Gifts may be made by check or credit card (Visa or MasterCard). Checks should be made payable to your Foundation of choice—either the City Hospital Foundation or Jefferson Health Care Foundation—and mailed to:

All donations are tax-deductible as allowed by law and are used to support patient care services at the hospital of your choice.
